James was born on October 2, 2001.  His unexpected death followed 11 days of preventable medical trauma. It started with the lack of full informed consent. It continued with faulty/broken medical equipment, failure to recognize declining condition, poor hand-off communications, a medication error, an unconsented surgery and ended with a hospital acquired infection, among other considerations.
